Applications are accepted Monday—Thursday 8:00 a.m. – 2:00 p.m. and Monday night 6:00 p.m. – 8:00 p.m .at the Town Clerk's Office


To file a passport, you will need:

Citizenship Evidence

» Current or previous passport

or

» Certified Birth Certificate containing:
Full name of applicant
Date of birth
Place of birth
Certification (Authorized seal)
Date of original record/registration date
Registrar's signature

or

» Certificate of Naturalization


Identification

Primary (Good Photo likeness required):

» Valid U.S. Drivers License

» Pilots License, State or Federal work ID card

» Current Student ID card (school or college)

» Military ID (Dependent ID)

» Passport (Foreign or U.S.)/Naturalization Certificate, Certificate of Citizenship

» INS Alien Registration Card (Green Card)/Employment authorization


Secondary (Submit at least FIVE of the below list):

» State ID cards (FIVE additional items must be submitted to support ID cards)

» Credit Cards, Medical Cards, Insurance Documents, Pay Stubs, Utility Bills,
Welfare/Benefit Cards, Leasing documents, Mortgage documents

» Marriage Certificate/Divorce Decree, Income Tax Records

» School Records, Yearbook with photo, Diploma/Degree, Technical School
certificate, GED certification, Vehicle Registration/Title, Military records (DD214, honorable Discharge)

Two IDENTICAL Passport Photos

2" x 2" full face, plain light background on acceptable photo paper.

Appropriate Fees

» Age 16 and over: $75 check or money order payable to the U.S. Department of State PLUS $25 to the Town of Boylston (may be cash or check)

» Age 15 and under: $60 check or money order payable to the U.S. Department of State PLUS $25 to the Town of  Boylston (may be cash or check)

» Expedite fee: An additional $60 fee to the U.S. Department of State is required when the passport is required in 2-3 weeks rather than the usual 5-6 weeks.
